Now You Can Attach Intake Forms to Any Booking You Create — Not Just Online Ones

October 23, 2026

If your team manually books appointments for clients, you've probably run into this: the online booking page collects all the right intake details, but when someone on your team creates a booking directly in the system, that information never gets recorded. The result is inconsistent records, missing context at the appointment, and extra back-and-forth before the client even walks in the door.

That gap is now closed. Attractzen's calendar and booking system lets you attach and complete intake forms right inside the appointment modal — so every booking, no matter how it's made, captures the same consistent client information.

What Changed in Attractzen

When creating a service or rental booking from within your Attractzen account, you'll now see a Booking Form section inside the appointment modal. From there, you can:

  • Attach a form in one click — your pre-configured form is already selected and ready to fill. You can also switch to any other available form from a dropdown.
  • Fill the form on the client's behalf — complete intake questions directly while creating the booking, logging everything in the same place.
  • Start with prefilled contact details — first name, last name, email, and phone are pulled automatically from the selected contact, saving time and preventing data mismatches.
  • Skip what doesn't apply — CAPTCHA fields, payment elements, and product selling blocks are automatically hidden when you're filling from the appointment modal. Only the fields your team actually needs are shown.

Attaching a form is optional. You can still create bookings without one if the situation calls for it.

How to Use It

  1. Open a new service or rental booking from the appointment modal inside Attractzen.
  2. Select the client contact.
  3. In the Booking Form section, click Attach Form.
  4. Fill out the form on the client's behalf and complete the booking.
  5. Open appointment details at any time to view the submitted form responses.
Note: Forms can be attached at the time of booking creation only. They can't be added or edited after the booking is saved.

Why This Matters for Your Business

Consistency in client intake isn't a nice-to-have — it's the foundation of a smooth client experience and an accurate record system. When one booking path captures detailed intake information and another captures nothing, your team is always playing catch-up.

With this update, your Attractzen calendar works the same way regardless of who made the booking or how. Whether a client booked online or your front desk created the appointment manually, the intake form gets filled and the information is logged in one place.

  • Every booking tells the full story — no gaps in intake data between online and manually created appointments.
  • Less prep work before each appointment — your team already has the details they need.
  • Faster booking entry — contact information prefills automatically, so you're not retyping what you already know.
  • Cleaner records — form responses are stored directly on the appointment, easy to review anytime.

If your business runs service appointments, therapy sessions, rental bookings, consultations, or any client-facing calendar — this upgrade removes one more gap between a great intake process and a reliable booking workflow.
